New ERS/ESGE/ESTS Guideline Advances Endosonography in Lung Cancer Diagnosis and Staging

The European Society of Thoracic Surgeons (ESTS), together with the European Respiratory Society (ERS) and the European Society of Gastrointestinal Endoscopy (ESGE), has published an updated clinical practice guideline on the use of endobronchial and oesophageal endosonography for the diagnosis and staging of lung cancer.

Accurate diagnosis and staging are essential for selecting the most appropriate treatment for patients with lung cancer. Developed by a multidisciplinary task force from the three societies, the guideline provides evidence-based recommendations on the use of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA), endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA), and EUS-B-FNA.

Based on a comprehensive review of the literature through April 2025 and using GRADE methodology, the guideline recommends endosonography over mediastinoscopy for mediastinal nodal tissue staging in patients with suspected or confirmed non-small cell lung cancer (NSCLC). Combined EBUS-TBNA and EUS(-B)-FNA is suggested whenever feasible to maximize diagnostic yield, while routine mediastinoscopy following a negative endosonographic assessment is no longer recommended.

The guideline also supports the use of endosonography for restaging after induction therapy, diagnosis of centrally located tumours adjacent to the airways or oesophagus, and assessment of the left adrenal gland. In addition, it highlights the importance of simulation-based training and confirms that EBUS-TBNA provides high suitability for PD-L1 testing in the era of precision oncology.

These recommendations reinforce the role of endobronchial and oesophageal endosonography as accurate, minimally invasive tools that have become central to modern lung cancer diagnosis and staging.
